Le NHS fonctionne selon un système à plusieurs niveaux : les recommandations du NICE priment sur la politique de passation de marchés du NHS England, les Integrated Care Boards (ICB) et les listes de médicaments de chaque hôpital, chacun ayant son propre calendrier,ses propres critères et ses propres décideurs. Sans relations existantes à aucun de ces niveaux, un nouvel entrant sur le marché doit s’y retrouver simultanément.
